Magnesium L-Threonate, Sleep and Cognition — a Completed Trial Awaiting Results
What was studied
A six-week randomised, double-blind, placebo-controlled trial at Barry University tested magnesium L-threonate in 81 healthy adults aged eighteen to forty-five. It finished in March 2026 and the results have not yet been published.
Two features make it worth watching. Biological sex and weight status were pre-specified as moderating variables rather than examined afterwards, and cognitive testing was done immediately before and after a twenty-minute mental fatigue induction protocol at each visit.
Testing under load rather than at rest
The fatigue induction is the design choice we find most interesting, and it connects directly to the mental fatigue series elsewhere in this library. Testing someone rested tells you about their ceiling. Testing them before and after a deliberate load tells you whether an intervention changes how far they fall.
That is a more demanding question and a more useful one. Several literatures reviewed on this site — tyrosine under stress, creatine under sleep deprivation — suggest that where compounds show effects at all, they show them under load rather than at baseline. A trial built that way is asking the question in the right place.
The registered primary outcomes are the Pittsburgh Sleep Quality Index, Flanker task reaction time measured around the fatigue protocol, and countermovement jump height — sleep, cognition and neuromuscular function in one design.
Pre-specifying the moderators
Declaring in advance that sex and weight status will be examined as moderators is good practice and rarer than it should be. Subgroup effects discovered after the fact are one of the main routes by which null trials get rescued into positive papers; naming them beforehand removes that option.
It also means a null main effect with a genuine sex difference would be reportable as such rather than as a failure, which is the sort of result the field needs more of.
What the existing evidence does and does not show
The most cited human trial of this compound tested Magtein — a magnesium L-threonate preparation — in a double-blind, placebo-controlled design. There is an important limitation in how it is usually described: the tested product was a formulation that also contained phosphatidylserine, vitamin C and vitamin D. Whatever it showed cannot be attributed to magnesium L-threonate alone.
A systematic review of magnesium and sleep health found the literature thin enough that its main contribution was noting that no one had critically reviewed it before. This new trial tests a single compound against placebo, which is precisely what the area was missing.
The chemical form matters
One point of relevance to anything sold as magnesium for the brain. Magnesium L-threonate was developed specifically because most magnesium salts have low bioavailability, and it is claimed to raise magnesium levels in the brain more effectively. Results obtained with it do not transfer to magnesium chloride, citrate or oxide.
Our own Clarity formulation uses magnesium chloride. Should this trial report a positive cognitive result, it would concern a different compound from the one we use, and we would say so.
Editorial Comment
Magnesium carries authorised health claims in the EU for normal psychological function and for the reduction of tiredness and fatigue, and those are the only statements available regardless of what this trial finds. A result about magnesium L-threonate and sleep quality would not extend them.
The trial is complete and unpublished. We are recording it now so that when results appear we have a note of what was pre-registered — the only reliable way to notice if the reported endpoints later differ from the planned ones.
